As recently requested I've made 2 additions to the CF-1.0 document:
1. Add azimuthal equidistant projection to Appendix F.
2. Add atmosphere_ln_pressure_coordinate to Appendix D.
All changes made to the CF-1.0 document since it's release are highlighted
and documented in the "Revision History" section at the end of the
document.
